The Zygoma Concept; Biomechanical Principles and Macro-Design

In the completely edentulous maxilla with sinus pneumatization and significant bone resorption, rehabilitation in the 1980s to early 1990s focused on bone grafting procedures to establish adequate bone volume for placement of implants.
Although good success rates were reported, the prolonged treatment plans with multiple staged procedures were barriers for patient’s willingness to proceed with recommended treatment plans.
For the past 20 years, a more simplified approach has been available for such patients. A paradigm shift considering the graftless approach, reduced treatment time, a single stage surgical reconstructions and immediate loading, eliminating the need for transitional dentures, has resulted in a higher degree of case acceptance by most patients.
The zygoma implant allows for a graftless treatment option for patients who present with residual alveolar bone in Zone I only as well as for patients with complete maxillary alveolar resorption. Placement of zygoma implants bilaterally combined with two to four conventional anterior dental implants allow for the fabrication of fixed implant supported prosthesis for these patients.
This presentation will discuss the critical biomechanical principles for treating patients with the zygoma implant. The macro-design of the zygoma implant will also be discussed as it is critical for use of the zygoma implant in the best possible manner.

One would be hard-pressed to escape the digital workflow taglines that have become exceedingly present in just about all facets of dental education. And with so many offerings surrounding this overarching topic, it may prove difficult, regardless of experience and desire of the clinician surgeon, to determine how best to introduce these techniques and technologies within their given practice setting.
This lecture is geared to those surgeons who have long felt confident in their free hand skillset or leaned heavily on analog approaches but would like to muster the courage to exit the comfort zone of “we do things pretty well already” and enter an exciting new phase of their implant careers with an unprecedented ability to deliver patient centered care. Digital and guidance is not a crutch, it is not for those with less surgical prowess, it is for those who strive for the highest level of care at both the patient and overall practice level.
