Approaching Dental Implants from the right angle: recognizing, preventing and treating misaligned aesthetic implants.

Misaligned implants in the aesthetic zone are not only frustrating to both clinician and patient but also can prove costly in both time and money. With modern technology and full digital flow, misalignment can be avoided. Technology, however, comes at a high cost, making it prohibitive in some cases. Many previously placed implants also present for new restorations with expectations of more favourable aesthetic results. When a misaligned implant presents for restoration, options should be carefully weighed before choosing a treatment option. The best treatment, of course, is to avoid misalignment through proper planning, pre-surgical preparation and the use of a digital flow. When misalignment occurred, minor deviations can often be corrected via prosthodontic and/or surgical means. Sometimes, however, such correction is not possible, and the only option is a surgical redo, which does not always yield a satisfactory result. According to guidelines, the decision to remove an implant should be made as soon as possible to avoid unnecessary tissue loss (which makes reconstruction more difficult) and plans for replacing the implant should be kept in mind at this initial surgery.

Key for long-term implant success is having sufficient hard and soft tissue volume and quality to establish a healthy emergence profile. In those sites with bone loss it will be crucial to re-develop the missing tissue in order to get predictable results. Several techniques have been described to repair sockets which are missing the buccal plate. In this presentation a modification of the Ice Cream Cone Technique will be discussed, focusing on case selection and indications/limitations. Step-by-step sequence from extraction and socket grafting to implant placement and restoration will be explained through clinical cases.

Cutting-edge techniques in bone augmentation using a customized allogenic block  

Despite the development of short and diameter-reduced implants, many patients still require alveolar ridge augmentation procedures to facilitate optimal implant stability and hence, clinical success. Digital technology now gives us the opportunity to visualize the bone defects by means of digital data based on CT or CBCT a customized allogenic block is designed to match each individual defect situation. This saves the patient an additional surgical site and pain, but also reduces the surgical time, since no manual adjustment of the block to the defect situation is necessary, with comparable clinical outcomes compared to autologous bone blocks. As one of the first users Dr. Oliver Blume will present clinical data and cases from his 5 years of experience using customized allogenic blocks giving insights into his surgery methods and incision techniques.

Placing a dental implant in the correct four dimensional position and within the future prosthetic envelop is considered as one of the crucial aspects of long term peri-implant bone and soft tissue stability in implant dentistry. It is unacceptable anymore to place an implant tilted buccally with the apex directed too palatally in order to achieve primary stability in cases with deficient bone volume as this will lead to a compromised and oversized clinical crown, minimal peri-implant labial bone thickness and gum recession.

Therefore, in cases with considerable bone defects such as one wall defects, no wall defects or even two wall defects with extensive mesio-distal gap, a pre-implant augmentation surgery is a vital prerequisite to allow correct implant placement in order to achieve satisfactory aesthetic and functional short and long term results. A variety of bone-grafting materials have been used, but autogenous bone blocks harvested from either extraoral or intraoral sites have been traditionally claimed to be the gold standard . However, allograft blocks are nowadays considered by a wide group of clinicians as a valid and well documented alternative which offers comparative results at a fraction of trauma, morbidity and risk to our patients.

In this webinar, Dr Maghaireh will present the clinical workflow of various types of allograft bone blocks used at his practice in Leeds- UK with more than 5 years follow up. He will also present bone grafting clinical tips and recommendations in addition to detailing advantages and potential drawbacks and discuss indications and contraindications of various types of allograft blocks.

Until now, a two-stage method has to be selected for the surgical treatment of 3-dimensional bone defects. Initially, the bone defect is to be reconstructed and implants are not inserted after a healing period of 6 months.

Dr. Bernhard Giesenhagen has developed a technique which allows bone augmentation and implantation to be performed simultaneously in a single procedure. The bone ring technique is applicable for almost all indications, including sinus floor elevation. The entire treatment time can be reduced by several months, a re-entry is not required. Application of allogenic bone rings can omit the harvesting of autologous bone and thus, simplify the surgical procedure.

Dr. Giesenhagen will present clinical data on the basis of 12 year experience to state the possibilities and limitations of the bone ring technique for vertical augmentation and the treatment of 3-dimensional bone defects.

This webinar will discuss biological principles in guided bone regeneration (GBR) and bone grafting. It will then, accordingly, introduce a suitable bone graft material and technique required to achieve predictable aesthetic results. 

Correct implant placement in the aesthetic zone and within the prosthetic envelope will, in 97% of the cases, necessitate GBR for better results and good long-term stability. GBR indications in the anterior region could range from fenestrations, dehiscences, thin labial walls, large three or two wall defects and ridge preservation cases. However, in cases with one wall or no wall defects, bone block grafting will be required for better and more predictable results.
