Following tooth extraction, a well-described process of wound healing takes place in the extraction socket.A series of physiological events occur, resulting in wound healing and bone regeneration within the socket. ITI Academy module: Immediate Restoration and Loading

The goals in implant dentistry are to provide safe and predictable treatment that restores health, function, and esthetics when replacing missing teeth. A further goal is to provide this treatment in a timely manner to satisfy patient needs for reduced treatment time. Once an implant is placed, there are three loading protocols that may be used. An implant loading protocol is defined as the time elapsed between implant placement and the attachment of the prosthesis so that the implant is brought into function. Conventional loading of dental implants is defined as loading 2 or more months after implant placement. This is a well-established protocol that allows for complete healing of bone and peri-implant soft tissues prior to prosthesis attachment. In an early loading protocol, the prosthesis is attached between 1 week and 2 months subsequent to implant placement. The implants are put into function during the later stages of bone healing but prior to complete healing of soft tissues. This results in a relatively reduced treatment time between implant placement and loading compared to the conventional loading protocol.In contrast, immediate loading of dental implants is defined as prosthesis attachment within 1 week of implant placement, including the day of surgery. In this case, the process of peri-implant bone and soft tissue healing occurs with the provisional prosthesis attached to the implant from the outset.

ITI Academy module: Timing of Implant Placement after Tooth Extraction

When patients request dental implant treatment or are identified as candidates for implant therapy, they often present with failing teeth that need to be removed or have recently been extracted. Once a tooth has been removed, dynamic physiological and dimensional changes take place during the healing phase that may influence the choice of the best time to place an implant. These dimensional changes are clearly visible in the clinical situation of the site eight weeks after tooth extraction. There are several time points following tooth extraction that may be considered for the placement of dental implants. Each time point has advantages and disadvantages. The clinician is in a unique position to be able to prescribe the time for implant placement following tooth extraction in order to maximize the advantages and to achieve successful long-term outcomes. An understanding of the factors that are important in determining the timing of implant placement is an essential part of the treatment planning process. These factors will be presented in this module.

With fewer steps, there is potentially also less risk of errors and discrepancies.However, digital impression is a fairly new approach and technology and the evidence base found in the literature is still lacking.This lecture presents aspects of the clinical application of intra-oral scanning and discusses why adopting modern treatment approaches deviating from the conventional modality can be challenging for practitioners, despite the pressure to stay in tune with modern technological developments. Use of 3D images is placed in the context of two aspects of precision: repeatability and reproducibility.Digital dental and maxillofacial imaging in 2D and 3D are compared and contrasted, including the principles of obtaining each image type and the most common sources of errors in linear and volumetric measurements. Superimposing DICOM and STL files in an appropriate software allows for precise planning of the implant treatment and production of the surgical stent. ITI Congress Lecture: Periodontitis versus Peri-Implantitis - Pathogenesis and Treatment Options

The aim of this lecture is to discuss similarities and differences between periodontal and peri-implant tissues, which affect the pathology, disease diagnosis and consequent treatment modalities.Clinically, peri-implantitis is associated with deepened pockets, bleeding on probing and/or suppuration and the loss of supporting marginal bone. Similar to periodontitis, the major etiologic factor is the microbial biofilm, and both implant components and implant restorations should be designed so that personal oral hygiene is feasible.Patients should be informed that peri-implant tissues respond to plaque accumulation in a similar way to periodontal tissues, and that disease may develop in the tissues around the implants, which would jeopardize their longevity.Adequate personal oral hygiene is essential and patients with a history of periodontal disease, in particular smokers and those suffering from diabetes mellitus, should be informed about the increased risk of peri-implant disease before implant placement. It is imperative that periodontal disease is treated before implant placement.

ITI Academy module: Immediate Restoration and Loading

The goals in implant dentistry are to provide safe and predictable treatment that restores health, function, and esthetics when replacing missing teeth. A further goal is to provide this treatment in a timely manner to satisfy patient needs for reduced treatment time. Once an implant is placed, there are three loading protocols that may be used. An implant loading protocol is defined as the time elapsed between implant placement and the attachment of the prosthesis so that the implant is brought into function. Conventional loading of dental implants is defined as loading 2 or more months after implant placement. This is a well-established protocol that allows for complete healing of bone and peri-implant soft tissues prior to prosthesis attachment. In an early loading protocol, the prosthesis is attached between 1 week and 2 months subsequent to implant placement. The implants are put into function during the later stages of bone healing but prior to complete healing of soft tissues. This results in a relatively reduced treatment time between implant placement and loading compared to the conventional loading protocol.In contrast, immediate loading of dental implants is defined as prosthesis attachment within 1 week of implant placement, including the day of surgery. In this case, the process of peri-implant bone and soft tissue healing occurs with the provisional prosthesis attached to the implant from the outset.

When patients request dental implant treatment or are identified as candidates for implant therapy, they often present with failing teeth that need to be removed or have recently been extracted. Once a tooth has been removed, dynamic physiological and dimensional changes take place during the healing phase that may influence the choice of the best time to place an implant. These dimensional changes are clearly visible in the clinical situation of the site eight weeks after tooth extraction. There are several time points following tooth extraction that may be considered for the placement of dental implants. Each time point has advantages and disadvantages. The clinician is in a unique position to be able to prescribe the time for implant placement following tooth extraction in order to maximize the advantages and to achieve successful long-term outcomes. An understanding of the factors that are important in determining the timing of implant placement is an essential part of the treatment planning process. ITI Congress Lecture: Periodontitis versus Peri-Implantitis - Pathogenesis and Treatment Options

The aim of this lecture is to discuss similarities and differences between periodontal and peri-implant tissues, which affect the pathology, disease diagnosis and consequent treatment modalities.Clinically, peri-implantitis is associated with deepened pockets, bleeding on probing and/or suppuration and the loss of supporting marginal bone. Similar to periodontitis, the major etiologic factor is the microbial biofilm, and both implant components and implant restorations should be designed so that personal oral hygiene is feasible.Patients should be informed that peri-implant tissues respond to plaque accumulation in a similar way to periodontal tissues, and that disease may develop in the tissues around the implants, which would jeopardize their longevity.Adequate personal oral hygiene is essential and patients with a history of periodontal disease, in particular smokers and those suffering from diabetes mellitus, should be informed about the increased risk of peri-implant disease before implant placement. It is imperative that periodontal disease is treated before implant placement.

ITI Congress Lecture: Does Patient Maintenance and Prosthesis Design Influence Bone and Soft Tissue Maintenance?

Preventive measures to avoid peri-implant infection are listed, and treatment strategies are outlined, including cumulative interceptive supportive therapy (CIST) for peri-implant mucositis and peri-implantitis. The available and increasing evidence for the importance of a zone of keratinized peri-implant mucosa to preserve healthy peri-implant tissues long-term is presented.
