This presentation will review the evolution of zygomatic implant treatment by examining the indications, surgical protocols and treatment outcomes. Zygomatic implant was first developed by Prof.  Per-Ingvar Brånemark to rehabilitate patients presenting with maxillary defects, congenital or acquired. Shortly after its introduction, zygomatic implant has been applied by the dental specialists to reconstruct severely atrophic maxillae due to extensive bone resorption after tooth loss, unsuccesful grafting procedures, or failed dental implant treatment. Although there is strong evidence to support that the zygomatic implants are effective for oral rehabilitation in the above indications, software and hardware aspects of the zygomatic implants have been evolving continuously.  The original protocol ad modum Brånemark with intra-sinus zygomatic implant trajectory is gradually replaced by the extra-sinus technique or the anatomy-guided approach. In addition, new designs of zygomatic implant with improved biological and mechanical features have been developed to enhance the treatment outcomes. In recent years, the digitalisation momentum of implant dentistry is increasing because of the corresponding technological advancement. Nowadays, complete digital workflow is feasible for zygomatic implant treatment including diagnosis and treatment planning, surgical and restorative execution. This presentation will review the evolution of zygomatic implant treatment by examining the indications, surgical protocols, and treatment outcomes.
