TERMS & CONDITIONS 

Private Practice Qualifications: Private practice qualification is based on January 1 through December 31, 2023. The qualification criteria are required to maintain membership status . Any benefits or services not utilised in the Program Year may not be carried over to the following year. A clinician must be part of a qualifying practice in the year prior to Program Year to enjoy member benefits and services.
Dental Service Organization and Group Purchasing Organisation Qualifications: For Dental Service Organization and Group Purchasing Organisation each practice location must meet the qualification criteria individually during the previous year to qualify for program membership and enjoy member benefits and services of the active program year. Any benefits or services not utilised in the Program Year may not be carried over to the following year.
The terms and benefits identified here are current as of November 2023. Straumann reserves the right to modify or cancel terms of membership or benefit program available through the program at any time and without notice. Straumann is not responsible for any claims or damages asserted by customers in reliance upon the terms and benefits of the program.
Straumann may decline to produce materials that do not comply with legal or regulatory requirements.
The Centres of Excellence Program will be reviewed annually and is subject to change. 
**This program is valid for 2024 and the final decision on admission rests with Straumann Pty Ltd.
*Straumann Group does not endorse any specific treatment provider and is not responsible for the quality of the treatment provided by the treatment provider. (Agreement T &Cs – clinician responsibility. It is the clinicians responsibility to ensure that they have the accurate skills and education to conduct the NeoArch treatments.
