CONSENT (“Consent”)
regarding the Recording Rights Grant to Institut Straumann AG,
Peter-Merian Weg 12, 4052 Basel, Switzerland (“Straumann”)

The undersigned (“You”) herewith agrees with the following terms and conditions:

Straumann acts as data controller for the personal data contained in or relating to the statement, photograph, film, or recording featuring you (the “Recording”). By agreeing to this document, you consent to Straumann and its affiliated companies processing such personal data for business purposes, including internal and external communication and marketing, and to publishing the Recording through current and future media and communication channels (including internet, print, video, and advertising formats, and social media platforms such as Facebook or Instagram), worldwide, including outside the European Economic Area.

You acknowledge that such publication necessarily involves the use of your image, voice, and other personal attributes and therefore grant Straumann and its affiliated companies an exclusive, worldwide, perpetual, irrevocable, and royalty-free licence to use, reproduce, adapt, edit, combine with other materials, publish, distribute, and make available the Recording, in whole or in part, in all media and formats now known or developed in the future, including through third parties, without financial compensation.

The Recording consists of a personal success story (a video and audio testimonial) in relation to the Straumann iEXCEL™ "Unleash your power" campaign. 

The Recording may be retained for publication purposes for up to fifteen (15) years and thereafter only to the extent required for legal, compliance, documentation, or archival purposes.

This consent is voluntary, and refusal or withdrawal will not result in any disadvantage unrelated to the use of the Recording. You may withdraw your consent to the processing of your personal data at any time with effect for the future. Withdrawal will not affect the lawfulness of processing or publications carried out prior to withdrawal. Following withdrawal, Straumann will cease further use of the Recording to the extent required by applicable data protection law, noting that complete removal may not be feasible for materials already lawfully published, disseminated, or archived.
